New histocompatibility general supervisor key personnel requirement

Notice of implementation: effective February 27, 2018
In June 2015, the OPTN/UNOS Board of Directors approved a rewrite of the histocompatibility bylaws that included the addition of the general supervisor to the list of laboratory key personnel. The implementation of the addition of this new requirement was delayed to allow for the development of a new key personnel application form, which then needed approval by the Office of Management and Budget (OMB). The OMB has recently approved the new general supervisor key personnel application form.
As part of the implementation of this bylaw, UNOS will send the OPTN general supervisor key personnel application to all OPTN approved histocompatibility laboratories. All approved histocompatibility laboratories will be asked to complete and submit the general supervisor key personnel application form within 30 days.
The effective date of the general supervisor key personnel requirement bylaw will coincide with the date that the Membership and Professional Standards Committee acts on all of the general supervisor applications received during the 30 day submission period. All OPTN approved histocompatibility laboratories will need to submit this form within the 30 day period to remain in compliance with the histocompatibility laboratory membership requirements. If UNOS receives the application after the deadline, UNOS cannot guarantee that it will be processed prior to the implementation date.

UNOS will implement the general supervisor key personnel requirement for histocompatibility laboratories on the following schedule:
November 2, 2017: UNOS Membership Analysts will send communication with general supervisory key personnel applications to all OPTN approved histocompatibility laboratories.
December 2, 2017: Due date for all general supervisor key personnel applications for consideration before the bylaw is implemented. If we receive your application after this deadline, we cannot guarantee it will be processed prior to the implementation date.
February 27, 2018: Effective date of general supervisory key personnel bylaw. Notice will be provided 30 days prior to the effective date.
