Resource form updated for verifying organ documentation and packaging UNOS' Department of Evaluation and Quality has recently updated a resource form for members to use in verifying the accuracy of documentation, labeling and packaging of transplantable organs. While this form is not required for member use, it summarizes in one location key information reviewed by site surveyors during OPTN site visits of OPOs. The revised form is available as a resource on the OPTN website. It provides additional space to document the use of extra blood vessels and tissue typing material. Members are encouraged to use the revised form effective September 1, 2012.
